چکیده

The value of nitroglycerin in determining the potential reversibility of asynergy was examined in 35 patients with coronary heart disease. Ventriculograms performed at rest and after sublingual nitroglycerin were analyzed for (1) location of asynergy relative to distribution of the 3 major coronary arteries and (2) severity of asynergy. Of the 41 hypokinetic zones, 30 (73%) improved following nitroglycerin. Of 28 akinetic zones, 16 (57%) improved (7 of the 16 segments becoming normal) following nitroglycerin. None of 7 dyskinetic zones showed change following nitroglycerin. Twelve patients were restudied following coronary bypass surgerv. There was an excellent over-all correlation between the segments that responded to nitroglycerin and bypass surgery in those segments with open grafts. Eighteen segments which improved following nitroglycerin had patent postoperative grafts, and in 15 segments there was a corresponding improvement following bypass surgery. Two segments which were unresponsive to nitroglycerin preoperatively had patent postoperative bypass grafts. Neither segment improved despite graft patency. In summary, sublingual nitroglycerin is useful in unmasking residual contractile ability in asynergic zones. A positive response to nitroglycerin appears to be predictive of corresponding beneficial effect from a coronary bypass graft. The data strongly suggest that the use of nitroglycerin to determine residual contractile abilitv mav be of considerable value in better defining the potential risks and benefits of coronary bypass surgery.

Elective operations for post-infarction left ventricular aneurysms.

During a two-year period (February 1973 to February 1975) 20 consecutive patients with post-infarction left ventricular aneurysm, seen at the Wessex Cardiac and Thoracic Centre, underwent aneurysmectomy with or without aorta-to-coronary artery saphenous vein bypass grafts, ventricular septal defect closure, or valve replacement. The diagnoses were established by clinical means, plain chest radi...

Long-term Survival of A Patient with Asymptomatic Left Ventricular Pseudoaneurysm after Acute Myocardial Infarction

An 82 years old man developed a left ventricular pseudoaneurysm after acute myocardial infarction when he was 72 years old. Coronary angiography showed left main and triple-vessel coronary artery disease. On left ventriculography, a tubular-like pseudoaneurysm was demonstrated that originated from the basal inferoposterior wall of the left ventricle. He underwent coronary artery bypass surgery ...

Relation between left ventricular filling pressure and angiographic findings in coronary heart disease. Ventriculography used as a stress test.

In 80 male patients with coronary heart disease maximum diastolic pressure in the left ventricle (LVMDP) (usually the top of the a wave), and 'post a' end-diastolic pressure (LVEDP) before and 2 and 3 minutes after ventriculography were correlated to angiographic estimates of left ventricular function (aneurysm, ejection fraction (EF), and to lesions shown by selective coronary arteriography us...

Cytokines and troponin-I in cardiac dysfunction after coronary artery grafting with cardiopulmonary bypass.

OBJECTIVE The association between cytokines and troponin-I with cardiac function after cardiac surgery with cardiopulmonary bypass remains a topic of continued investigation. METHODS Serial measurements, within 24h following surgery, of tumor necrosis factor-alpha, its soluble receptors, and troponin-I were performed in patients with normal ejection fraction undergoing coronary artery bypass ...
